Glucagon-Like Peptide-1 Receptor Agonists and Risk of Systemic and Ocular Vascular Complications in Patients With Type 2 Diabetes and Diabetic Retinopathy.

A large propensity-matched cohort (173,216 adults with type 2 diabetes and pre-existing diabetic retinopathy — a high-risk group often excluded from trials) drawn from the TriNetX network. GLP-1 receptor agonist use, with semaglutide and tirzepatide among the six agents in the exposure definition, was associated over two years with lower risk of a broad range of vascular complications: myocardial infarction, heart-failure exacerbation, ischemic stroke, amputation, acute kidney injury and dialysis, and — notably for the retina — less progression to proliferative diabetic retinopathy, fewer retinal vein occlusions, and less neovascular glaucoma. Reassuringly for the ongoing NAION debate, this study found no association with NAION or retinal artery occlusion. Two caveats anchor the reading: semaglutide and tirzepatide are pooled within the class exposure, so agent-specific effects cannot be separated, and the observational design leaves room for confounding by indication despite matching. This large study looked at whether GLP-1 medications are linked to fewer complications in a high-risk group: people with type 2 diabetes who already have diabetic eye disease (retinopathy), who are often left out of clinical trials. Using a big health-records network, researchers compared more than 30,000 people taking one of these drugs (including semaglutide and tirzepatide) against a closely matched group who were not, following them for two years. The people on GLP-1 drugs had lower rates of a wide range of serious problems — heart attacks, heart-failure flare-ups, strokes, amputations, and kidney injury — and, importantly for the eyes, less worsening of retinopathy, fewer retinal vein blockages, and less of a severe glaucoma. The study found no link, in either direction, with a rare optic-nerve condition called NAION that has been debated for these drugs. Two cautions: the study grouped several drugs together, so it can't isolate semaglutide's or tirzepatide's individual effect, and because it observed patients rather than randomly assigning treatment, other differences between the groups could contribute to the results.
