Bayesian rare-event methods are the right tool here, and the credible intervals tell the story: the atrial-fibrillation estimate (OR 2.20, 95% CrI 0.81-6.75) is compatible with anything from no effect to a large one. The any-arrhythmia signal just crosses significance and warrants monitoring rather than alarm. Event counts this low mean longer trials and pharmacovigilance data will settle the question, not this analysis — a point the authors themselves make.
