A single case, so no generalizable conclusions — but Prader-Willi syndrome is a population where appetite-driven obesity is uniquely severe and therapeutic options are thin, which gives the observation some weight. The within-patient comparison (tirzepatide succeeding where a GLP-1 mono-agonist fell short) is suggestive of the added GIP component mattering, though switching effects and dose differences can't be ruled out. Small PWS trials of incretin therapies would be the logical follow-up.
