Adjunctive Treatments for Patients with Type 1 Diabetes Using Automated Insulin Delivery Systems. A Narrative Review.

Kitsios K, Trakatelli CM Diabetes technology & therapeutics. 2026.

Editor's note

This narrative review draws on a thin evidence base — the authors themselves describe the underlying data as limited, mixing small randomized and observational studies. Off-label GLP-1 use in type 1 diabetes is a live clinical question, and the early signal here is directionally encouraging on weight, insulin dose, and time in range. Larger and longer trials are needed before the safety picture, especially around ketoacidosis, can be considered settled.

Plain-language abstract

A narrative review of add-on medications for people with type 1 diabetes using automated insulin delivery systems, finding that weekly semaglutide or tirzepatide was linked to weight loss, lower daily insulin doses, and more time in glucose target range without added hypoglycemia or ketoacidosis risk.
