The target-trial-emulation design and extensive sensitivity analyses are stronger than typical database work, but this remains retrospective claims-style data, where diagnosis capture and residual confounding can move hazard ratios of this size. The 0.82 hazard ratio against other GLP-1 agents is modest; the comparison against non-incretin drugs is the more convincing signal.
