Tirzepatide's dedicated cardiovascular outcomes trial has not yet read out, so this carefully built claims-database study is the best available preview — and it comes from a Harvard group whose methods have been benchmarked against randomized trials. A hazard ratio of 0.68 for major cardiovascular events would put tirzepatide in the same territory as semaglutide's SELECT result, and the negative-control checks came back clean. It remains observational, and the unexpected infection-related findings deserve replication, but this is a substantial signal ahead of the definitive trial.

In US insurance data covering nearly 53,000 people with type 2 diabetes and existing heart disease, starting tirzepatide was linked to a 32% lower rate of heart attack, stroke, or death within a year compared with a neutral comparator drug. Serious infections and overall mortality were also lower.
