In male Sprague-Dawley rats with surgically induced spinal cord injury, intranasal Semax (labelled here ACTH(4-10)/Pro-Gly-Pro) was associated with higher anti-inflammatory cytokine levels (IL-4, IL-10, IL-13) in injured tissue versus saline, most clearly at the 3-hour timepoint and in milder injuries. This is a small preclinical study with very short follow-up (3 and 6 hours) and immunohistochemical endpoints only, with no functional or recovery outcomes. It adds to the anti-inflammatory strand of Semax's rodent literature but cannot speak to clinical spinal-cord-injury outcomes, and the authors themselves call for dose and outcome studies. These are preclinical findings; human data are needed before clinical conclusions can be drawn.

Plain-language abstract

Spinal cord injury causes lasting loss of movement and sensation, and part of the later damage comes from inflammation. This study asked whether Semax, a peptide with reported anti-inflammatory and nerve-protective properties, could raise levels of calming (anti-inflammatory) immune signals after such injury. Working in male rats, surgeons created mild or severe spinal cord injuries using clips, then gave one group Semax as a nasal spray and another group salt water as a placebo. Tissue was examined 3 and 6 hours later. Rats given Semax generally showed higher levels of anti-inflammatory signals (IL-4, IL-10, IL-13), with the strongest effect in milder injuries and at the earlier 3-hour mark. The authors say more work is needed to find the best dose and to see whether this affects actual recovery. Because this was a short rat study, it does not show a benefit in people.
