This is a secondary, exploratory analysis of two randomized (2:1) trials examining tesamorelin's effect on skeletal muscle quality (density) and quantity (area), restricted to visceral-fat responders (n=193 vs 148 placebo). Using CT segmentation, researchers reported that tesamorelin was associated with significantly greater increases in the density of four trunk muscle groups and in the area of several muscle groups versus placebo, after adjustment for baseline and treatment arm. As an exploratory secondary analysis limited to responders and using imaging surrogates, it is hypothesis-refining; effect sizes are small in absolute terms. It complements the meta-analytic finding that tesamorelin is associated with increased lean mass, adding that muscle density (quality) may improve too. Long-term functional significance remains unstudied.

Beyond shrinking deep belly fat, could tesamorelin also affect muscle? Researchers reanalyzed two randomized trials, focusing on people whose deep belly fat responded to the drug, and used CT scans to measure the density (a marker of muscle quality) and size of trunk muscles. Compared with placebo, tesamorelin was associated with denser muscle across four trunk muscle groups and with modest increases in the size of some muscles, after accounting for starting differences. The changes were small in absolute terms. The authors note that among people who lose significant deep belly fat on tesamorelin, muscle area and quality tended to improve, but they call for more research on whether this makes a difference in daily life and whether it applies to people without HIV. This was an exploratory look at imaging measures rather than a test of real-world strength or function.
