Efficacy of ipamorelin, a novel ghrelin mimetic, in a rodent model of postoperative ileus.

A preclinical rat study of ipamorelin for postoperative ileus, examining efficacy rather than merely mentioning the peptide — and notably comparing single versus repeated dosing. A single intravenous dose of ipamorelin (or GHRP-6) shortened time to first bowel movement but did not improve cumulative stool output, food intake, or weight gain at 48 hours. Repeated dosing of ipamorelin, by contrast, significantly increased all three. The dosing-schedule contrast is the substantive finding: it suggests sustained ghrelin-receptor stimulation may be needed for meaningful gut and appetite effects, consistent with the compound's known mechanism. These are rodent data, however, and the parallel human Phase 2 ileus trial elsewhere in the literature did not show a significant benefit — a reminder that rat motility signals do not reliably translate. Weight this as a mechanistic, hypothesis-generating result. Human data are needed before drawing clinical conclusions about ipamorelin for gut-motility problems.

This study in rats tested whether ipamorelin, a compound that mimics the hunger hormone ghrelin, could ease postoperative ileus — the temporary slowing of the gut after abdominal surgery. Male rats had surgery involving handling of the intestines, then received ipamorelin, the related peptide GHRP-6, or a saline placebo, given either as a single injection or repeatedly (four times a day for two days). The researchers tracked how long until the first bowel movement, plus total stool output, food intake, and body weight over 48 hours. A single dose of ipamorelin or GHRP-6 shortened the time to the first bowel movement but did not change overall stool output, eating, or weight. Repeated dosing of ipamorelin, however, significantly increased all three. The researchers concluded that ongoing treatment with ipamorelin after surgery might ease ileus symptoms. Because this was done in rats, the results are preliminary and point to a possible mechanism rather than proof of benefit in people; human studies would be needed to know whether it helps patients.
