A preclinical efficacy study — a rat model of postoperative ileus — and a genuine investigation of ipamorelin's promotility action rather than a mere mention. After surgery-induced gastric slowing, a single intravenous dose of ipamorelin significantly sped gastric emptying (52% of the meal remaining vs 78% in vehicle controls, p<0.05), approaching non-surgical levels. In isolated stomach-muscle tissue, ipamorelin and ghrelin reversed the surgery-induced loss of contractility, and the authors attribute the effect to ghrelin-receptor activation of cholinergic excitatory neurons. This is mechanistically coherent and consistent with ipamorelin's known ghrelin-receptor agonism. But these are rodent findings; animal-to-human translation for gut motility is unreliable, and the human counterpart — the Phase 2 ileus trial elsewhere in this literature — did not show a significant benefit. Read this as a hypothesis-generating mechanistic signal, not clinical proof. Human data are needed before any clinical conclusions can be drawn.

This study, done in rats, tested whether ipamorelin — a synthetic compound that activates the ghrelin receptor — could speed up a sluggish stomach after abdominal surgery, a common problem called postoperative ileus. Fasted male rats underwent surgery that included handling of the intestines, which slows stomach emptying. Some then received ipamorelin by injection into a vein, others a placebo. Stomach emptying was measured using a tagged test meal. In the placebo group, 78% of the meal was still in the stomach; with ipamorelin, only 52% remained — close to the 44% seen in rats that had no surgery — a statistically significant improvement. In a second experiment, stomach muscle tissue from operated rats contracted poorly, but adding ipamorelin (or natural ghrelin) restored the contractions. The researchers concluded ipamorelin speeds stomach emptying by activating the ghrelin receptor and triggering nerve signals that make the muscle contract. Because this was done in rats, the findings are preliminary; they suggest a mechanism but do not prove the drug works in people, and human studies would be needed.
