A large multi-site cohort in a population — people with HIV — largely absent from the semaglutide liver trials, which is what makes this notable. The pre/post design without a comparator group leaves room for regression to the mean, and FIB-4 is a blood-based estimate of fibrosis rather than biopsy or imaging. Still, the dose-response-like pattern by baseline fibrosis severity is consistent with the general-population evidence, including the phase 3 MASH data.

Among 1,850 people with HIV who started semaglutide across 10 US clinical sites, those with moderate-to-advanced liver fibrosis scores saw those scores decline over about seven months on the drug, with the largest improvements in the advanced-fibrosis group.
