Semaglutide's approval anchors the incretin arm of a field that until recently had nothing licensed at all. The review's substantive contribution is the phenotype argument: liver-centric subsets with fast fibrogenesis may respond differently from cardiometabolic subsets driven by insulin resistance, which would help explain heterogeneity across trials. Combination regimens and biomarker-guided selection are framed as the direction of travel, not established practice.

This Nature review surveys the drug landscape for MASH, the progressive form of metabolic fatty liver disease, noting that the thyroid-hormone-receptor agonist resmetirom and the GLP-1 agonist semaglutide are the first agents to win accelerated approval for non-cirrhotic MASH with fibrosis. The authors argue MASH is really several overlapping diseases, so future therapy will likely need to be matched to patient subtype and possibly combined.
