Editor's note

This is an extension phase of the randomised ADJUST-T1D trial, so the comparison groups carry over real randomisation - a strength for a discontinuation question that is usually studied observationally. The sample is very small (16 who stopped semaglutide versus 22 on placebo), and the differences, while statistically adjusted, sit close to the significance threshold. Type 1 diabetes remains an off-label setting for this drug class, which makes durability data like this worth tracking even at this scale.

Plain-language abstract

Adults with type 1 diabetes and obesity who had been taking semaglutide alongside an automated insulin pump were followed for 12 weeks after stopping the drug. Their continuous glucose monitors showed less time in the target glucose range and more glucose swings than in people who had been on placebo. Measures of low blood sugar did not differ between the groups. The authors read the results as evidence that the glucose benefits seen during therapy fade once the drug is withdrawn.
