Disproportionality analysis of spontaneous reports detects reporting patterns, not risk — EudraVigilance has no denominator, and semaglutide's media profile alone can inflate its report counts relative to dulaglutide. The suicidal-ideation confidence intervals are very wide (ROR 6.49, CI 1.57 to 26.81), which signals sparse counts. The authors are appropriately hedged and align with the regulators' position that no causal link is established. Useful as a vigilance signal for people with existing ophthalmic or neuropsychiatric vulnerability, and nothing more than that.
Plain-language abstract
Analysts mined the European drug-safety reporting database for 2020 to 2025 and compared adverse-event reports across semaglutide, liraglutide and dulaglutide. Eye disorders were reported disproportionately more often with semaglutide than with the other two, and suicidal ideation reports were disproportionately higher for semaglutide and liraglutide than for dulaglutide. Regulators have not established that these drugs cause either outcome.
