A PRISMA-guided systematic review of whether GLP-1 agonists influence alcohol use, an area of intense interest given the drugs' action on reward pathways. It pooled five studies (n=49,892), three RCT-based analyses and one large cohort, with formal risk-of-bias assessment. The finding is measured: semaglutide and dulaglutide were associated with modest reductions in alcohol consumption and craving, with significant improvements on selected behavioral outcomes, while exenatide showed no overall effect (only subgroup signals), and the cohort study found small but significant AUDIT-C reductions after initiation. That heterogeneity of agent and outcome is the key caveat, alongside the fact that most included data are secondary analyses of trials designed for other endpoints, not purpose-built alcohol-use trials. Objective biomarkers (PEth, breath alcohol) were reported in only a few studies. The authors are appropriately restrained: the signal is real but preliminary, and adequately powered dedicated RCTs are needed before any role in alcohol use disorder can be defined.

This is a systematic review that combined the best available studies on whether GLP-1 medications affect how much people drink. These drugs act on the brain's reward system, which is why researchers suspect they might influence alcohol use. Following standard review methods, the authors searched major databases and included five studies covering nearly 50,000 people, three based on randomized trials and one large observational study. The results were mixed but leaned in one direction. Semaglutide and dulaglutide were linked to modest reductions in alcohol consumption and craving, with meaningful improvements on some behavioral measures. Another drug, exenatide, did not show an overall effect, with hints of benefit only in certain subgroups. The large observational study found small but real reductions in a standard alcohol-use screening score after people started these drugs. A few studies used objective measures like blood or breath alcohol tests and also saw reductions in some settings. The authors caution that the evidence is still limited and inconsistent, and that larger, purpose-built trials are needed before these drugs could have a defined role in managing alcohol use disorder.
