Editor's note

Nationwide register studies like this are how trial results get stress-tested against everyday practice, and here the oral formulation broadly held up: glycaemic, weight, lipid, and liver-enzyme improvements all moved in the expected direction. Two caveats deserve equal billing. Discontinuation was high, with nearly half off the drug by one year, and the endpoint samples shrink sharply from the full cohort, which can flatter the averages among those who persisted.

Plain-language abstract

Finnish national registry data on over 7,000 adults with type 2 diabetes who started oral semaglutide showed meaningful one-year improvements: HbA1c fell about one percentage point, average weight dropped roughly 8 kg, and cholesterol, triglycerides, and the liver enzyme ALT all improved. Only about 55% were still on the drug at one year.
