The Effect of Semaglutide on Quality of Life in Adults With Overweight or Obesity: A Brief Systematic Review and Meta-Analysis.

Editor's note

A sobering counterpoint to the weight-loss headlines: across the STEP program, semaglutide's average effect on patient-reported physical function was statistically real but below the threshold for a change patients would actually feel. Effects were smaller still in participants with type 2 diabetes. The authors' framing is careful — the drug improves these outcomes, but expectations about quality-of-life gains should be calibrated. All included trials come from the manufacturer's own development program, which limits the evidence base's independence.

Plain-language abstract

This analysis pooled quality-of-life data from four STEP trials of weekly semaglutide 2.4 mg, covering about 4,200 adults with overweight or obesity. Semaglutide did improve patient-reported physical functioning versus placebo, but the average gain — 1.71 points on a standard survey — fell short of the 3.7-point threshold considered noticeable to an individual patient. In short: the weight loss is large, but the average day-to-day functional improvement people report is modest.
