The optic neuropathy question has followed semaglutide since the 2024 observational reports, and this adds a mechanistic hypothesis rather than epidemiological weight. The hydration measurement is the novel element: a documented 1.9 litre decline in total body water alongside stable fat mass gives a plausible physiological bridge between the drug and an ischaemic optic event. It remains one patient, and the authors themselves ask for prospective studies before any inference about risk.

A single case report describes a 28-year-old man with type 2 diabetes who developed non-arteritic anterior ischaemic optic neuropathy in his right eye four to five months after starting semaglutide. Imaging confirmed the diagnosis and also found buried optic disc drusen in both eyes; the affected eye was anatomically short while the unaffected eye was highly myopic and longer. Serial bioimpedance showed total body water falling by 1.9 litres over seven weeks while fat mass was preserved. The authors propose a hypothesis-generating two-hit model in which crowded optic disc anatomy plus sustained low hydration together precipitate the event.
