This phase 2b open-label trial reports that semaglutide is associated with improved MASLD severity in people with HIV, a population with limited prior data. The single-arm design without a placebo control and reliance on a metabolomic surrogate rather than biopsy temper the strength of the conclusion. Regard it as encouraging evidence in a specific population that warrants controlled confirmation.

In the single-arm SLIM LIVER trial, 24 weeks of low-dose semaglutide was associated with significant improvement in fatty-liver disease severity among people with HIV, as measured by a non-invasive metabolomic panel. The study also showed the panel can monitor disease during GLP-1 therapy.
